T\u00e9l\u00e9chargez-le, remplissez le. Si vous avez r\u00e9pondu \"NON \u00e0 toutes les questions, cochez les deux cases correspondantes sur le syst\u00e8me d'inscription. Sinon, rendez-vous sur la page <a href=\"https:\/\/unicon20.fr\/health\/medical-certificate\/\" data-type=\"page\" data-id=\"1455\">Certificat M\u00e9dical<\/a> vous en aurez besoin pour participer \u00e0 l'Unicon 20. Si vous avez d\u00e9j\u00e0 fournit un certificat m\u00e9dical, il n'y a pas besoin de remplir le questionnaire.<\/p>\n\n\n\n<p>* les monocyclistes fran\u00e7ais adh\u00e9rents de l'UNSLL ont d\u00e9j\u00e0 rempli ce questionnaire.
