Questionnaire (under 18)

To attend Unicon 20, minors* (under 18 years old the 26th of July 2022) must fill out a medical questionnaire. Please download it, then fill it. If you have answered “NO” to all questions, then proceed to the registration software and check the corresponding box. Otherwise, please go to the Medical Certificate page, you will need it to attend Unicon 20. If you have already provided a Medical Certificate, there is no need to fill the questionnaire.
