Please download it, then fill it. If you have answered &#8220;NO&#8221; to all questions, then proceed to the registration software and check the corresponding box. Otherwise, please go to the <a href=\"https:\/\/unicon20.fr\/health\/medical-certificate\/\" data-type=\"page\" data-id=\"1455\">Medical Certificate<\/a> page, you will need it to attend Unicon 20. If you have already provided a Medical Certificate, there is no need to fill the questionnaire.<\/p>\n\n\n\n<p>* French riders members of UNSLL have already filled this questionnaire.
